Teressa Vaughn is an Advance Care Planning (ACP) Consultant for Sharp Healthcare where she assist patients with advanced illness and/or advanced age complete their advance healthcare directive, discuss goals of care as well as caregiving options that meet their need. She also provides community education on advance care planning, POLST, and the End of Life Option Act (EOLA). She is an active member of the San Diego Coalition for Compassionate Care (SDCCC).
Prior to her work as an ACP Consultant, she was a hospice chaplain for several years. She received her Master of Practical Theology from the Kings Seminary (now The Kings University) and retired from the Army National Guard after 25 years of service. She also received her Master of Health Administration with a concentration in Gerontology from the University of Phoenix and is a licensed Residential Care Facility for the Elderly (RCFE) Administrator with the state of CA.
She is active in her community as a member of the Alpha Kappa Alpha Sorority, serves on the board of Planned Parenthood Pacific Southwest, is an active member of her church and a full-time caregiver for her mother who has advance Alzheimer’s dementia.
